To obtain a copy of your medical records at Mass General, you must submit a written, signed and dated authorization form. Download the English (PDF) and Spanish (PDF) version of this form.
To avoid delays, your signed release form must include the following patient information:
- Name (or patient’s name)
- Date of birth
- Social security number
- Medical record number (your “Blue Card” number or Mass General patient number)
- Dates of service and treatment at Mass General
- Your address or the address to which you are sending medical records
- Your telephone number
- Your signature

Medical records can include medical history, pathology, radiology, lab reports and operative reports, which will be sent by mail to the address you provide. To obtain copies of radiology images or films, please contact the Radiology Imaging Services Center directly at 617-726-1798. Please note that Mass General does not provide birth or death certificates. Please be aware that a fee may be associated with your request, and you should allow up to 30 days for processing.
If the patient is a minor or is not competent to give consent, the signature of a parent, guardian or other legal representative is required. For recently deceased patients, the request must be accompanied by documentation proving you are the patient’s next of kin (death certificate with name of INFORMANT or probate document assigning administrator). Additional authorization may also be required for release of specifically protected or privileged information. Contact the Release of Information Unit at 617-726-2361 with specific questions.
For urgent medical record copy requests, patients can visit our medical records copy office conveniently located on the hospital's main campus on the 8th floor of the Founder's building.
