Our annual post-graduate course in general thoracic surgery -- the first such course offered in the United States -- continues to fulfill its mission for the 40th season. The objective of this course, which will be held May 26-27, 2011, is to provide an in-depth review of selected topics in general thoracic surgery. Emphasis will be on current principles of management and surgical technique.

This course, which runs from April 6 to April 10, 2011 at the Fairmont Copley Plaza Hotel in Boston is appropriate for clinicians in the areas of endocrinology, diabetes, reproductive endocrinology, family practice, general practice, and pediatric endocrinology.

11/19/2010: Topics In Contemporary Obstetrics And Gynecology

The course content will be divided roughly equally among the four subspecialty areas in obstetrics and gynecology; reproductive endocrinology and infertility, gynecologic oncology, urogynecology andmaternal-fetalmedicine.

11/07/2010: Trauma and Critical Care Symposium

The symposium, which will take place Nov. 7-9, 2010, provides an ideal mix of the traditional standard-of-care principles with cutting-edge, forward-moving practices. It explores issues related to surgical technique, resuscitation, inflammation and infection, respiratory care, nutrition, prehospital care, and ethics.

10/29/2010: Surgery of the Thyroid & Parathyroid Glands

Preoperative work up of both thyroid and parathyroid surgical disease will be discussed with an emphasis on fine needle aspiration and preoperative parathyroid localization imaging options. Surgical techniques will be reviewed, including recurrent laryngeal nerve monitoring, parathyroid identification and preservation, and intraoperative PTH analysis during parathyroid surgery. Minimally invasive surgical techniques will be thoroughly discussed. Preoperative in-office sonography will be reviewed. Molecular genetic analysis for medullary carcinoma and advances in oncogene research as it applies to clinical thyroid and parathyroid disease will be presented. Surgical philosophy regarding bilateral thyroid surgery, surgical treatment of the neck, and surgical treatment of invasive tracheal disease will be presented. Recognition and treatment of postoperative complications and new advances in their treatment will be reviewed. Panel discussions will be centered on management of the difficult thyroid and parathyroid cases and will invite audience participation.

For most patients, schizophrenia is a life long disorder that requires maintenance treatment with antipsychotics and psychosocial support to avoid inpatient hospitalizations. However, the administration of antipsychotics comes at a cost with respect to medical comorbidity for most patients. In addition, recent large controlled trials that compare second generation antipsychotics with first generation antipsychotics have called into question the superiority of second generation antipsychotics (other than clozapine) over first generation antipsychotics with regards to efficacy. In reality, many patients remain symptomatic and may be at risk for hospitalization, chronic disability and suicide. A systematic approach to the selection and dosing of antipsychotic agents must take place after patients fail to respond to initial treatment interventions. For most patients, consenting to the use of an antipsychotic medication is an individualized choice that balances sufficient efficacy with acceptable side effects, a process that must be negotiated with patients. Clozapine remains an underused option for patients who have only had a partial response to first line antipsychotics, despite good evidence for its superior efficacy.
